Mandatory Reporting of Suspected Child Abuse
This training must be completed online. Please register at the link below. Return the results to your Sport Coordinator or to Linda Vogel at the following email address: lindav.abvm@gmail.com. This is a one-time requirement.
Mandatory Reporter Acknowledgement Form
This form is needed to provide proof to the state of Pennsylvania that the above Mandatory Reporter training has been completed and also to acknowledge that the volunteer is now, or may be considered in the future, to be a Mandatory Reporter. Volunteer Disclosure Form
This form is needed to provide proof to the state of Pennsylvania that the volunteer has not been named as a perpetrator of a founded report of child abuse within the past five (5) years as defined by the Child Protective Services Law. Protecting God's Children
This training is conducted in person. Please register at the link below under On-Site Trainings. Locations and times are provided as well as the number of slots open for each session. Please provide a copy of the completion certificate to your Sport Coordinator or to Linda Vogel at the following email address : lindav.abvm@gmail.com. This is a one-time requirement.
Note that by completing this course, you will also have covered the previously required Information and Communication Technology Addendum.
Consent Form - Posting Pictures / Videos of Minors
In order to protect the privacy of youth, permission must be obtained, in writing, from the parent or guardian before sharing / posting pictures or videos of minors online. Download the Consent Form and fill out / return it with your registration form any time you're registering your child for a sport.
